Admission
The Guilford House admits and discharges close to 900 patients annually - approximately 70% emanate from the Yale New Haven Hospital campus that is located 12.8 miles from the facility. Further, approximately twenty percent of the facility’s patients reserve rooms based on planned medical procedures. Therefore, the balance of the facility’s patient admissions is the result of an unplanned illness, accident and ill fortune. While hospitalized, the patient and hospital discharge planner will make a determination that short-term rehabilitation is the advised next step in the patient’s recovery. A referral generated by the hospital with all pertinent patient medical information will be sent electronically to The Guilford House. The facility admission, nursing and rehabilitation staff will evaluate the medical documentation and notify the hospital if the patient is clinically appropriate. Transportation to The Guilford House is arranged by the hospital either by family, chair car or ambulance.
Length of Stay
The facility’s average patient length of stay is 16 days – knee and hip replacement surgery anywhere from 5 to 14 days while other surgeries, falls, strokes, cardiac, respiratory and other medical complications may require a longer stay. Regardless, we are committed to utilize all our resources to return each patient to the community swiftly and safely.
